Kagetsu Tohya is a sequel to Tsukihime, featuring the same main character, a high-school student named Shiki Tohno. The game's main story is entitled Twilight Grass Moon, Fairy Tale Princess; completing certain events while playing through this story unlocks other playable short stories, which are grouped together as Ten Nights of Dream and deal with other characters from Tsukihime.

In the principal story, Shiki becomes a victim of an accident, after which he finds himself trapped in one day, forced to re-live it over and over again, like in the movie Groundhog Day. The only way to continue his normal life is to contact the mysterious creator of the dream in which Shiki finds himself imprisoned.

Similarly to its predecessor, the game is essentially a visual novel, its gameplay consisting entirely of following the story and making choices to influence its course from time to time. Due to the game's premise, the visual novel mechanics are slightly modified: rather than continuing a story throughout the whole game, the player repeatedly makes choices within the same day of the hero's life. After the day is over, the player's choices are saved, and he returns to the beginning of the day again, with the consequences of his previous choices further influencing its events.
