Tōhō: Kaikidan No rest for the wicked - that's something Reimu Hakurei, a young girl guarding the medieval Japanese Hakurei shrine, has understood during her eventful life. Demons from a parallel dimension have chosen her shrine as an entry to our earth, from which they begin their invasions. This time they appeared in the mountain cave, and Reimu must stop them before they conquer the entire world. But this time, she is not alone...

The fifth entry in the T?h? series follows the traditional vertically scrolling shooter template first introduced in the second game. Enemies, level design, collecting weapon upgrades, using a limited amount of powerful "bombs" - all the elements from the other T?h? games are present here. Like in the previous entry, the witch Marisa, Reimu's rival, is a playable character; in T?h?: Kaikidan, two more characters "join the club", the demonic (though looking like young cute girls) Mima and Yuka, who starred as boss enemies in the fourth game.
