The Lost Cases of 221B Baker St. Like its predecessor, The Lost Cases of 221B Baker St. is a hidden object game where the player assumes the role of the famous Victorian detective created by Arthur Conan Doyle. There are sixteen cases to be solved organized in groups of four. The cases within a group can be completed in any order, but all must be finished to unlock the next group.

Most of the time, the objective is to find and click on all the items listed at the left side of the screen, on many locations related to the case and filled with assorted paraphernalia scattered throughout the scenery. A time limit can be turned on at the start for extra challenge. The hint button is Holmes' pipe at the bottom-left corner, showing the position of one of the required items when activated. There are five hints per case, but extra ones can be found and collected as pipes laying around the scenes. A magnifier can be selected, replacing the cursor and enlarging portions of the screen to make the search for small items easier.

Mini-games appear on certain locations, where the player has to reassemble shredded notes, place a series of gears on a machine, click on the differences between two similar images, move stone blocks inside a box to create an exit path for a slab, and complete other similar challenges. A recurring game has to be solved at the end of each case, where portraits of the suspects have to placed on a grid with columns and lines describing personal traits like glasses and scars, to find out who is the criminal.
